I think the issue was more that he took her past health history and made the insta-conclusion that it was all about hygiene instead of a long told background of UTIs, surgery, and happened well before she could wipe herself. He also focused on her weight as being a reason she /couldn't/ possibly have good health hygiene. He was behaving poorly as a doctor and deserves no defense.
